Academic Misconduct
Please submit all instances of academic misconduct for in-person or online classes with this form. Be sure to indicate the correct school when submitting the report.
General Incident
Use this form to report general incidents occurring on campus including; concerns, accidents, hazing, and student conduct violations. The report will be routed to the Office of Student Conduct. The form is only monitored M-F during business hours.
Sexual Harassment or Misconduct (Title IX)
Please use this form to submit all concerns relating to Sexual Misconduct including; Sexual Harassment, Sexual Assault, Dating or Domestic Violence, and Stalking. This form is monitored M-F during business hours.
Student Complaint
Use this form to submit a formal complaint once you have exhausted existing academic departmental processes. Please refer to NS Student Complaint Policy for more information.
Harassment and Discrimination
Please use this form to submit concerns relating to harassment and discrimination. Harassment is defined as any unwelcome behavior that creates a hostile, intimidating, or abusive environment and discrimination is the unfair treatment of individuals or groups based on protected characteristics. This form is monitored M-F during business hours.
Wellness Referral
Please use this form to submit a concern or refer a student to our Student Wellness team. Your report will be directed to the team for appropriate action and is monitored M-F during business hours.
SANS
The Student Absence Notification System is in place to review documentation related to emergency/extenuating circumstances outside your control, connect you with campus resources for support, and notify your instructors about the absence.
University Police Report
Use this form to submit a crime report with University Police Services.
Non-91ÌÒÉ« Concern
If you are not currently a 91ÌÒÉ« Faculty, Staff, or Student and would like to submit a report to the institution please do so via the attached link. Your report will be routed to the appropriate staff for review.
Campus Security Authorities
Faculty and staff serve as Campus Security Authorities (CSAs) — folks on campus who have gone through training and are able to ensure that reports are made to the appropriate channels.
